What do I need to submit?
The applicant must submit the following information to the licensing authority:
- the prescribed application form
- the prescribed fee - (cheques etc. should be made payable to Chorley Council)
- where alcohol is to be supplied, the application must state the name and address of the person (who must hold a personal licence) whom the applicant wishes to be the designated premises supervisor (DPS) and, if not the applicant, the DPS must complete the consent form. Special arrangements may apply in the case of church or village halls, etc.
- an up-to-date scale plan of the premises - view information to be shown on the plan.
Note: A person commits an offence if he/she knowingly or recklessly makes a false statement in or in connection with an application.
